PHOENIX -- Arizona transportation officials said a new system is to blame for some people getting the wrong picture on their driver's permit.

Local TV news reported that Arizona Motor Vehicles Department director Stacey Stanton said she learned Tuesday that there have been five cases of people receiving a license or ID with someone else's photo and signature.

Stanton said the agency transitioned last week to a new centralized system and that a team of employees are examining what could be causing the glitch.

Officials don't think it's a systemic issue and that applicants' personal information is still safe.

Anyone who finds an error on their driver's license or identification card should immediately call the department.
